Terms and Conditions of
Participation in BCUC's Med Club:
- Your
participation is conditional upon all payments being current at the time of the
visit.
- The
I.D. card of the participant being treated must be presented at the time that
services are requested.
- Treatment
of minors will not be rendered without a parent or legal guardian present.
- Certain
conditions may require a referral to a specialist or a hospital. Med Club is NOT A HEALTH INSURANCE
PROGRAM and DOES NOT cover services that are not provided by Bee Caves Urgent
Care.
- Rates
are subject to change upon thirty days notice.
Services Not Included in Med Club Participation:
- Hospitalization
or treatment at other facilities
- Preventative
treatment/maintenance care, immunizations, or routine pregnancy care
- Physical
therapy or rehabilitation
- Terminal
care
- Maintenance
care for chronic disease (diabetes, thyroid disease, chronic renal failure,
etc.)
- Mammograms,
MRI's, CAT scans, PET scans, or Doppler Studies
- Intravenous
nutrition
- Treatment
of life threatening events or major trauma
- Percutaneous
intravenous catheters (PICC) lines or other central lines
- Medications
or Durable Medical Equipment
